             2020 Application Dates and Fees
       • Application Opens: March 16, 2020 (Application fee of $500 for all 
        applications, Specialty and Sub-Specialty).
       • Practice time for Practice Pathway counted through June 30, 2020
       • Application Closes: June 30, 2020
       • Late Application Opens: July 1, 2020 (Late fee of $500 applies, in 
        addition to the $500 application fee).
       • Late Application Closes: July 15, 2020
       • Documentation Deadline: July 31, 2020
       • Appeal Period: 30 days from the date of the decision notification 
        ($500 fee)

       General Eligibility Requirements
       • Current certification by any of the ABMS Member 
        Boards
       • Medical degree or equivalent
       • Medical licensure: current, unrestricted and valid in 
        US or Canada in all jurisdictions where the physician 
        has license(s)
       • Training or experience in Addiction Medicine

                  Helpful Hints for the Practice Pathway 
        •   All applications are reviewed by an ABPM Addiction Medicine board certified physician. The reviewer will consider the 
            CV and the practice descriptions listed in the application. All applications decisions are rendered by ABPM board 
            certified physician reviewer(s).
        •   ABPM staff will conduct a verification of an applicant’s medical degree, medical license and ABMS board certification 
            status. Verifications are conducted through the American Board of Medical Specialties (ABMS) and/or the Federation of 
            State Medical Boards (FSMB).
        •   ABPM staff as part of the verification process will seek public records through the individual medical state license 
            portals as needed. Explanations of any prior and/or current action is recommended to avoid processing delays. 
            Applicants applying with a current or prior PHP, may be required to provide a letter of compliance from the agency.
        •   Applications for the Practice pathway are required to have current ABMS member board certification. 
        •   Applicants may choose a practice description from the dropdown menu. The menu includes Direct Patient care practice 
            activities that are required to meet the practice pathway requirement of 480 hours of Direct patient care in Addiction 
            Medicine. The menu also lists practice activities for Research, Training, and Administration activities. 
        •   All activities selected require a detailed description and the number of hours engaged in the activity (on average) per 
            week. 
        •   Applicants should avoid double counting the same activity for the same practice. 
        •   Broad-based, professional activity with significant Addiction Medicine responsibility
               •   Clinical care including prevention and treatment of individuals at risk for substance use disorders (minimum 480 
                   hours) 
               •   Teaching, research and/or administration                             The American Board of Preventive Medicine, Inc.®  A Member Board

        •   1,920 hours of relevant practice in Addiction Medicine at the subspecialty level
               •   Completed over at least 24 months
               •   Need not be continuous, but must be within 60 months prior to application for certification
             Practice Pathway: Non-ACGME-Accredited Fellowship
             •   Successful completion of at least 12 months of fellowship training accredited by the American 
                 College of Academic Addiction Medicine (ACAAM, formerly ABAM and then The Addiction 
                 Medicine Foundation)
                   •  Documentation of the training curriculum
                   •  Description of applicant training experience
                   •  Verification by Program Director
             •   If fellowship training is less than 12 months, this time in training may be applied to the “Time 
                 in practice” track
             •   Applicants applying through the Practice pathway with a non-ACGME accredited Addiction 
                 Medicine must provide a complete description of the program. 
             •   A verification of the Fellowship will be required from the Program Director. 
             •   Applicants with additional practice experience in Addiction Medicine, may add the practice 
                 time may to the practice section of the application. 
             •   Applicants with an ACGME-accredited Addiction Medicine Fellowship should apply 
                   through the Addiction Medicine Accredited Fellowship pathway.
