Instructions Tab


Completing this form satisfies the requirement that University Service Providers ("USPs") develop a goods and services Rate Calculation Schedule for their users, as provided in Policy 1410 University Service Providers, Section 1410.4 Rates and Charges. It details the USP's cost buildup for the identified time period.

NOTE: In some instances, it may be necessary to attach additional details to this form (e.g., "consumables" might be an annualized number in the "Expense Report" tab; the USP is expected to attach additional detail specifying the derivation of that number).
NOTE: ALL DOLLARS in this form can be whole dollars, and should be ANNUALIZED amounts, except on the "Summary" tab where usage is separated by customer type.




USP Cover Sheet
The "USP Cover Sheet" tab provides the basic information of the USP. This includes USP name, number, type, COA, contact information, and Lead Administrator approval





Complete relevant mellow-yellow cells.
1 Enter the USP name, USP ID, and USP Description. These should match your USP registration.
2 Row 6: Select the USP type from the drop-down menu. Only Specialized Service Facility ("SSF") and Recharge Center ("RC") USPs are required to complete this rate calculation worksheet.

3 Row 7: Select YSM or Non-YSM from the drop-down menu. This field drives the "all other" fringe rates on the "Salary Calc & Services Offered" tab.
4 USP COA (Cost Build-Up for Service Provided): Enter all appropriate COAs used by the USP.
5 Contact information of individual completing form: Enter the name, department, and phone number of the individual completing the form.
6 The Lead Administrator must certify that they have reviewed and approved the Fully-Costed Rate(s) and Customer Rate(s) using Form 1410 FR.02A University Service Providers: Rate Calculation Worksheet - Lead Administrator Certification Page. The Certification Page must be submitted simultaneously with this completed Rate Calculation Worksheet to ISP@yale.edu.

Salary Calculations and Services Offered Tab
The "Salary Calc & Services Offered" tab provides the services offered by the USP as well as the salary, appropriate fringe rates, and effort related to each service. This tab must be filled out in all cases with the list of services offered.





Complete relevant mellow-yellow cells.
1 Enter the names of the services that are provided by the USP. If there are more than 5 services, unhide additional rows. The current form allows up to 55 services.

2 For each person, enter the person's name in Row 13, select the appropriate category (M&P, C&T, Faculty, Part Time, Casual, Student). Input the full annual salary amount in the input box in Row 71. In the next step you will allocate what portion of their annual time relates to each respective service. In the Salary Comments field in Row 72, please explain any portion of time not covered by the USP's services. Also explain the funding source for other time/effort.


3 Enter the % of effort the individual will devote to each service; this number can be less than 100% and can be a fraction (Note: the form will round the number up, but calculations will use the actual number entered). The total of these cells should be equal to or less than 100% and is conditionally formatted to indicate if it is greater than 100%.


4 Repeat for each individual. If there are more than 4 individuals, please unhide additional columns. The current form allows up to 70 employees.
5 The "All Others" fringe rate will populate based on the data entered above.





NOTE: If the cells in the fringe rate return a "FALSE" error, please be sure to complete Step 3 in the USP Cover Sheet section.






MEI Utilized by USP Tab
The "MEI Utilized by USP" tab will show the equipment that is used by the USP and the pertinent details related to each piece of equipment. Additionally this tab will use key data by MEI item to calculate non-federal depreciation expense that may be included in the USP allowable costs.
Note that relevant information for the USP equipment is found in the Assets for University Service Providers - Yale report in Workday.





Complete relevant mellow-yellow cells.
1 Enter the description/type of equipment (Column B). Schedule has 200 rows available that can be unhidden.
2 Enter the MEI tag number (Column C), which should match what is in Workday.
3 Jurisdiction is the owner of the equipment (Column D).
4 Enter the acquisition date (prior to 2018), or date placed in service (beginning in 2018) (Column E) and the acquisition value (Column F), both of which should tie to the date and value, respectively, in Workday.

5 Next, enter the percent of the MEI that was purchased with sources excluding federal sponsored awards (FUND02), which should tie to Workday values (Column H).

6 Enter the fiscal year of purchase/year placed in service for the equipment (column I), followed by the useful life in years that the piece of equipment will be depreciated (column J). The final depreciation year (column K) will populate based on this information. Use acquisition date if acquired prior to 2018 and use the placed in service date if acquired 2018 or later.


7 Depreciation for current fiscal year (Column L) is linked from the "Depreciation Table" tab, which will be updated in the next step.
8 If the equipment is used for more than one service, an allocation table is available to allocate the equipment depreciation expense to more than one service, with the ability to allocate to 55 services. First identify the relevant services to which the depreciation will be allocated in Columns N-BP, Next, input the allocation percent's beginning in Column N for each specific equipment item. The total allocation percent in Column BQ must equal 100% and is conditionally formatted to highlight in red if it does not. Please provide an explanation in Column BR if it is below 100%.



9 Columns BS through DV will automatically calculate the depreciation expense allocation to each service. Depreciation by MEI item and Service does not automatically populate the "Expense Report" tab at this time. The data must be linked manually to Column L in the "Expense Report" tab. There is a check figure in cell M3219 to ensure all depreciation is correctly brought into the "Expense Report" tab, which is conditionally formatted to show up red if the numbers do not agree.








Half-Year Convention: The University adopted half year convention for depreciation expense beginning in FY2018. This method assumes that the business asset is in service for one-half of its first year, irrespective of the actual placed in service date. The remaining half year of depreciation is recorded in the final year of the business asset's useful life. Prior to FY2018, an asset was placed in service by the department when it was received, and the depreciation began in that month.







Depreciation Table Tab
The "Depreciation Table" tab recalculates annual depreciation and is agreed to the source of record, which can be done using the Assets for University Service Providers - Yale report in Workday. Refer to the note above about the half-year convention. All equipment placed in service during the current fiscal year will record half of the annual depreciation amount.

1 Columns B-H will auto-populate from the "MEI Utilized by USP" tab.

2 Columns I-V will auto populate with the depreciation amounts needed for the "Fund Balance" tab, including only the non-federally funded portion of depreciation. These columns can be collapsed so that they are out of view.

3 Column W picks up the amount of the current fiscal year depreciation from the table and multiplies it times the non-federally funded portion. This is linked to the "MEI Utilized by USP" tab in Column L.

4 In Columns Y-CM, record the annual depreciation amount for each respective service. This should be a formula using the cells from Column E / Column H (acquisition amount / useful life) for those years when the asset is in service, which is adjusted in the year of acquisition and year of disposition in accordance with the policy described above. If you have any questions about how to calculate the depreciation, please reach out to ISP@yale.edu.






NOTE: All equipment placed into service in this fiscal year will be recorded using the formula (acquisition amount / useful life * 50%) in Column AM for FY23.






Expense Report Tab
The "Expense Report" tab provides information for all of the expenses (both allowable and unallowable in separate sections) related to the USP for the upcoming fiscal year. It divides estimated future recoverable costs by estimated future usage for each of the goods and/or services provided.

Costs: USPs estimate their future recoverable costs by reviewing their current recoverable costs, analyzing the previous year's activities and reasonableness of those costs, and, using knowledge of their activities and business projections for the next fiscal year, creating an estimate for the next fiscal year's costs.

Usage: USPs estimate future usage expected during the fiscal year in which the calculated rate will be charged. Estimated usage should represent the volume of units expected to be used in the fiscal year, not the highest potential output of the USP.

1 Begin by entering the location for each service, including the building and room (Column B).
2 Enter the anticipated number of requested units for each service in the fiscal year (Column D). Expected usage should include both internal and external users, regardless of rate charged. Even if no rate will be charged for a good or service (e.g., via an offered discount), all usage should be included in the estimated total. Overestimating usage will result in a deficit, while underestimating usage will result in a surplus. Typical units of measure for usage include hours of machine time, hours of labor, number of users, pounds, or liters, but may vary based on the specific good or service provided by the USP.



3 Enter the % of Service Capacity, which shows the expected capacity of the service and whether it is under- or over-utilized (Column E).





Allowable USP Expenses
4 Enter allowable consumable expenses (Columns G & H ). These would include any supplies required to provide the service (e.g., pipettes, alcohol, beakers, reagents, chemicals, etc.). Enter a brief description of the consumable items (Column G) and the annual cost of the supplies (Column H). Description should not be too general (i.e. "supplies"). If a consumable item is shared by more than one service, enter the item on a separate row, include the annual price, and indicate how the cost is distributed between services that share the item (e.g., expense split proportionally between services A, B, and C). Similarly, for general lab supplies that are used by all services, the annual cost can be split proportionally between all services. Exclude unallowable expenses and exclude passthrough items invoiced to the customer.
Add an additional tab with supplemental information, as necessary, to demonstrate how the amount of consumables were derived.






5 Labor (Column I) is carried over from the "Salary Calc & Services Offered" tab. This is the salary plus fringe at the federal rate.
6 Enter the equipment description utilized by the USP for each service (Column J) by linking it from the "MEI Utilized by USP" tab. This information is found in the "MEI Utilized by USP" tab Column B. There are 5 rows available to enter equipment. If you need more, please unhide additional rows. There are 40 rows available for each service.


7 On the same row from Step 6, enter the equipment asset number (MEI tag #) in Column K by linking it from the "MEI Utilized by USP" tab. This information is found in the "MEI Utilized by USP" tab Column C.

8 On the same row from Step 6, enter the % Usage of Equipment, which indicates the amount of time the equipment is used for that specific service. If a piece of equipment is needed for more than one service, split the percent use proportionally between the services (Column L). This information should be linked from the "MEI Utilized by USP" tab using the appropriate percentage from Columns N - BQ.
9 Enter the non-federal portion of the depreciation costs (Column M) by linking from the "MEI Utilized by USP" tab from Columns BU-DX. This is the precent that has been allocated to the respective service (which was calculated in Step 9 of the "MEI Utilized by USP" tab instructions above). If the equipment is used by more than one service, the depreciation costs must be split proportionally between the services.
10 Enter the annual cost of service contracts (Columns N & O). The service contract cost and description needs to be indicated on the same row as the piece of equipment in Column J. Include in the service contract notes (Column N) the vendor, contract number, and the covered time period, and the amount of the agreement in Column O. If the contract is multi-year, provide the total contract price and indicate the amount being charged to the USP account for the current fiscal year. Remember to allocate across services if the equipment is also allocated across services.



11 Enter any miscellaneous allowable expenses by briefly describing the miscellaneous expense(s) in Column P and the amount in Column Q. Please add an additional tab with details behind miscellaneous expenses; include ledger account, spend category, and any other details that may be relevant. Please make sure the unallowable costs are not included in this section.


12 Enter any surplus/deficit carryforward based on the USP's federal fund balance calculation in column R.  (See "Fund Balance" tab for relevant information.) Subsequent prior year fund balance surpluses in excess of 60 days must be included after review by YSM or Central Rate Reviewer unless another plan for inclusion is approved by the USP Operating Committee.  Subsequent prior year fund balance deficits may be included after review by YSM or Central Rate Reviewer.  

Per Procedure 1410 PR.02 University Service Providers: Rate Calculations, Section 4B:

"USP costs, in all years but the USP’s first year of operation, must also be adjusted by any surplus, and may be adjusted by any deficit, in the account fund balance, as described below:

• Surplus: Any federally-calculated surplus in excess of 60 days of operating expenses in the account fund balance must be included as a reduction in the recoverable cost estimate, thereby reducing future rates charged. If, however, the USP’s federally-calculated surplus is much larger than 60 days of operating expenses, the USP need not fully eliminate that surplus in a single year. In such instances, the USP may request approval from their appropriate Rate Reviewer and the USP Operating Committee to implement a 2- to 4-year reduction plan.

• Deficit: USPs with a federally-calculated deficit may include that deficit in the subsequent year’s recoverable cost estimate with prior approval from the USP’s Rate Reviewer and lead administrator. Ability to roll-forward this deficit is limited, however, to the immediate subsequent year only.

These adjustments to the accumulated current costs for each good or service produce the current recoverable USP costs for that good or service."




















Non-Allowable USP Expenses
13 The fringe differential is carried forward from the "Salary Calc & Services Offered" tab (Column AC). Fringe differential is the difference between the federal fringe rate and the "All Others" fringe rates.

14 The space column (Column AD) is only applicable to SSF USPs. Contact ISP@yale.edu for questions on whether or not this applies to your USP.
15 Any additional non-allowable expenses can be included in columns (AE to AH). Please enter the description of the non-allowable expenses in Row 5. Please add an additional tab with details behind the additional non-allowable expenses and include ledger account, spend category, and any other details that may be relevant.






Passthrough Items Tab
The "Passthrough Items" tab is to provide a representative list of additional goods and/or services provided by this USP that are not included in the cost buildup for any other goods and/or services offered.  The USP obtains these goods and/or services from external suppliers and resells them at cost. The USP does not charge internal users for personnel (effort) and/or other administrative costs related to the provision of goods and/or services or impose any additional charges for these passthrough items. The charges associated with these goods and/or services are commonly known as “passthrough charges.” The list is not intended to be exhaustive of all specific passthrough charges, but should provide a representative categorization of known passthrough charges as of the date of this submission. Additionally, these costs should not be included in the consumables number included on the "Expense Report" tab in Column H.

The original invoice (i.e. record of the original charges that are then passed on to the customer) should be retained in accordance with University documentation retention policy. The USP needs to be able to provide support that the cost of the passthrough items to the customer is at the price they paid.





Summary Tab
The "Summary" tab provides a summarized version of the costs by service and the rates that will be charged by the USP for Internal, External For-Profit, and External Non-Profit customers.

1 Each service will feed over from the previous tabs. If needed, unhide the number of rows that correspond with the number of services provided by the USP ("Salary Calc & Services Offered" tab, column A).

2 Divide the number of anticipated requests for each service, as indicated in the "Expense Report" tab, between the 3 types of customers: Internal, External For-Profit, and External Non-Profit. The estimated usage by each user group should be entered into column D in the appropriate section. There is an added check below the 'External Non-Profit' group to ensure the total usage from the expense tab matches the quantities entered on the summary sheet (see Rows 195 - 249).



3 Next, enter the billing unit into Column E. The terms entered into Rows 10 - 64 will populate the corresponding rows in the External For-Profit and External Non-Profit sections. Typical units of measure for usage include hours of machine time, hours of labor, number of users, pounds, samples, slides, assays, or liters, but may vary based on the specific good or service provided by the USP.


4 Columns F&G contain the calculated rate based on the values entered in the "Salary Calc & Services Offered" tab and the "Expense Report" tab. Column F is calculated with the depreciation costs entered in the "Expense Report" tab. Column G excludes the depreciation costs, permitting calculation of the USP's actual operating costs (sum of Column G x # of anticipated requests for each service).


5 Enter in Column H the proposed Customer Rate for the fiscal year.

For internal customers, the rate can NOT exceed the maximum calculated rate to deliver one unit of the service (amount in column F).

For external for-profit customers, the proposed rate should be, at a minimum, the cost to deliver service + 30% (in accordance with the Provost Office Guidance for External Sales, included at the link below). For those Schools and Units that allow it, an additional administrative rate may be included. If the rate being charged to users is less than the calculated rate (1.30 x cost-to-deliver the service), an explanation should be provided in the comments column.

For external non-profit customers, the rate should be, at a minimum, the cost to deliver service +30%. If the proposed rate is less than this amount, an explanation should be provided in the comments column.

7 Columns J & K show the rate difference between the Fully-Costed Rate and the proposed Customer Rate. There is conditional formatting on these cells such that, if a profit is being made on the Internal Customer Rates versus the Fully Costed Rates including Depreciation, the cells will turn red indicating that it is not allowed. For the two external Customer Rates, the cells in Column I will turn red if the amount is less than the minimum amount that should be charged (i.e., cost + 30%).

Columns L & N show the estimated revenue surplus/deficit based on the projected usage and proposed rates.
8 Write comments in Column O for further explanations, as needed. If columns L and N project a deficit, however, state in the comments for that row who (i.e., which funding source(s)) will be covering any deficit that occurs.

9 Columns Y - W are used to demonstrate if the external rates are above the fully costed rate + 72.2% for tax purposes.





Fund Balance Tab
The federal fund balance is a required form to prove that the USP is not making a profit, and if a profit is somehow generated, that the costs are being offset in future years to reduce rates. Surpluses in excess of 60 days of operating expenses must be included in a future year's rate, and deficits may be included. Please work with the USP's designated Rate Reviewer if a surplus exists or if the USP wishes to include a deficit in future rates.

1 Complete the form, starting in FY2009, to arrive at the cumulative fund balance. The amount is always 2 years in arrears (e.g., for FY23 rates, the form should be updated cumulatively through FY21 actuals). If this cumulative schedule was prepared in prior year service rate calculations, please copy the existing information into this tab and update for actual operating results for FY21. Please email ISP@yale.edu with questions on this calculation.


2 Run the SOA YTD Actuals with 3 Year Trend - Yale report in Workday to obtain the fund balance for the current fiscal year. This report will show you the beginning balance, revenue, expenses, transfers, and ending balance.

3 Run the Detailed Transactions - Yale report in Workday to obtain all of the revenue and spend categories related to the USP's journal source. This report will provide all of the details necessary to complete the fund balance form.

4 In cells B6 - B8 of the Fund Balance tab, enter the COA segments used to run the 2 above reports. Note that both reports should use the same COA segments.
5 In the top section of the form (Rows 10-13), enter the revenue (by type: LA50002 internal revenue, other internal revenue, external revenue, and all other revenue which will be excluded from the federal calculation). Add the respective ledger account(s) to Column A and the ledger account description(s) to Column B. All revenue should be included in this section. You will back out exclusions in the next section.


6 Next enter the total operating expenses (Row 16). The total operating results (Row 18) should match the Workday data, excluding transfers.

7 Enter any applicable revenue exclusions, including USP discounts and/or premiums and surcharges for both internal and external USP sales (Rows 20-22). Other Internal Revenue exclusions include funding for the USP and other External Revenue exclusions include endowment income. These should be added in Row 23, with the ledger account added in Column A and ledger account description in Column B. These revenue exclusion amounts must not be included in calculating the federal surplus or deficit, as they do not relate to the cost of providing the goods and/or services. Note you should input premiums and surcharges as a positive number, and discounts as a negative number.




8 Complete the unallowable section next. This section includes expenses to provide the service(s) that were charged to the USP account but are unallowable for federal purposes. Refer to the "Unallowable Costs" tab for a listing of spend categories that should be excluded. The most common unallowable costs have been included in Rows 28-30, which may or may not apply to your USP. Add additional unallowable costs for your USP in Rows 31-58 (you may unhide additional rows).
Note that fringe benefits at the University fringe benefit rates and equipment purchases with a cost ≥ $5,000 are unallowable and must be excluded. The last section of the schedule, Fringe Calculation at Federal Rates, will recalculate fringe benefits at the federal rates by fiscal year based on salary amounts input by Fringe Code S/H/P.





9 The "other adjustments" section will add various costs that are not recorded in the University's management books, but are federally allowable. These include any fringe benefits at federal negotiated rates and MEI depreciation for equipment that was not purchased with federal sponsored funds (FUND 02). The fringe amount is linked to the section below the form that allows you to calculate the fringe benefits at federal negotiated rates.



10 In Row 62, the depreciation amount is linked to the "Depreciation Table" tab for FY20 and FY21, where the equipment depreciation is being recalculated for the non-federal portion. For FY09 - FY19, please input the amounts from prior Fund Balance calculations. Note that if you have full depreciation information included on the Depreciation tab in prior years and it agrees to the prior Fund Balance calculations, you can hyperlink from Columns G-Q on the Depreciation tab to the Fund Balance tab Row 62.
It is less likely that the USP will include Building & Fixed Equipment depreciation, allowable interest expense, and other property adjustments (Rows 63-66). Contact ISP@yale.edu if you believe these should be included in your form.





11 The next section begins with a calculation of the Federal Surplus/(Deficit) for the individual years and cumulatively. It is unusual for a transfer into the Federal Fund Balance, so please work with the Rate Reviewer for your USP if this occurs. The row "Ending Federal Fund Balance" is the row that will dictate whether or not a carryforward will need to occur. This will not tie to the book balance, as it excludes certain expenses and includes other expenses that are not charged directly to the USP COA.



12 The beginning book fund balance should tie directly to the SOA report for the USP from Step 2. Fund balance transfers (Row 74) should include any fund transfers on the USP books. The ending book fund balance should tie to the ending fund balance in Workday for each year, which ties to the SOA report.

13 In order to calculate the Fringe Calculation at Negotiated Federal Fringe Rates, enter the salary data in Rows 82-84 using the Detailed Transaction report from Step 3.

14 The last section in Rows 97-99 calculates the Federal Fund Balance Surplus to be included in the current year service rate calculations. Policy allows a surplus to be reduced by 60 days of operating expense. If Row 69 has a surplus, the reduction of the surplus by 60 days of operating expenses will automatically recalculate for you.
