IMPORTANT: HEALTH INFORMATION FORMS FOR ALL FAMILIES 
       
      Attached to this letter, parents will find three important health related forms. If you have not 
      already done so, I would ask that you review these and complete them, as they pertain to your 
      child or children. 
      Student Health Certificate Update 
      This form is required for all new and returning students. It provides us with up to date OHIP 
      numbers and version codes, as well as medication and health updates. Please ensure that current 
      contact information is provided for use in the event of an emergency. If you have not already 
      done so, please complete this form and return it to the TCS health centre by fax (905-885-
      8860) or email to jkedwell@tcs.on.ca. 
      Seasonal Influenza Immunization Consent Form 
      This fall, the School will be offering flu shots on campus, once again. The consent form must be 
      completed for each student requiring a flu shot. If you wish your child to receive a flu shot this 
      year, please complete this form and return it to the TCS health centre by fax (905-885-8860) 
      or email jkedwell@tcs.on.ca. 
      Health Insurance Coverage for International Students 
      This letter explains details about health insurance for international students. For returning 
      families, a reminder that our TCS coverage does not automatically renew each school year; the 
      School must receive an email request each year to enroll students in this insurance plan. Please 
      contact me by email at jkedwell@tcs.on.ca if you wish your son/daughter to be covered 
      with this insurance for the 2017-2018 school year.

            Health Insurance Coverage for International Students 2017-2018 
   Trinity College School has once again arranged for health coverage to be made available for our 
   international students with insurance provider Destination Travel.  
    
   The Destination Travel International Student Hospital and Medical Elite Plan coverage can be obtained or 
   renewed by email request to Jennifer Kedwell, R.N. at jkedwell@tcs.on.ca. The premium for an annual 
   policy for the current school year will be $650. 
    
   A reminder: This coverage does not automatically renew for returning families; you must renew your 
   coverage for 2017-2018 by contacting Ms. Kedwell. 
    
   We strongly recommend that you have health coverage in place for your son or daughter before they arrive 
   at the School in September. We realize that some parents may have coverage through their own places of 
   employment or will purchase other insurance for their children before they leave home. Students are under 
   no obligation to participate in the School’s plan; however, should you wish to enrol your son or daughter, 
   please contact the school health centre and we will arrange for your child to be enrolled. Please be reminded 
   that we do not administrate third party insurance coverage through the School. Students not covered by our 
   school plan will be responsible for paying medical bills through their student accounts. All original receipts 
   will be provided for reimbursement purposes.  
    
   Students who live overseas but are Canadian citizens are eligible to apply for OHIP (Ontario Health 
   Insurance Plan) coverage. They may apply immediately upon arrival in Canada, but will serve a three month 
   waiting period before the coverage becomes effective. A pro-rated version of our international student plan 
   may be purchased to cover this period of waiting time. If your child is already covered by OHIP, please 
   disregard this letter. However, if your child has an OHIP card with a pending expiry date, please make 
   contact with the school health centre to arrange a renewal appointment at the local OHIP office.
