T.H. 15 both directions Sample Project Description: Unbonded Concrete Overlay, Culvert Lining, Bituminous Surfacing Project Identification: ☒ Project Manager ☒ Construction Project Supervisor Sample Letting Date: November 2020 (construction limits are due two years before letting) Sample Construction Dates: May through September 2021 Scope of work: Contractor will provide the following to the State in accordance with Exhibit B, Scope of Work and the indicated schedule: 1. Coordinate with District and Central Office functional groups and local governments to identify potential scope items and determine the project scope, schedule and budget. For purposes of developing a contract bid, assume that item one above led to determination of the following scope: · Remove existing bituminous · Unbonded concrete overlay; 1.5” Passrac; 7.5” concrete (up to 9” grade raise); 28’ top (3’ inside shoulder, 2x12’ lanes, 1’ of outside shoulder) · Reclaim 4” shoulders; add gravel for grade raise; paving outside shoulder 4” bit 8’ wide · Unbonded concrete overlay on all ramps in project area; tie in at ramp terminal · Widen shoulder (as gravel) for truck parking at top of on ramps · Reconstruct roadway under overpasses · Reconstruct roadway at tie ins on each end and at bridge over Watonwan river · Reconstruct side road to tie in to new elevations · Striping · Rumble strips · Grading inslopes to maintain 1:4 inslope · Culvert extensions at 18 culverts · Repair (or replace) 6 condition 3 and 4 culverts · Riprap basins as needed · Replace triple 10’ CSP culvert with precast concrete box culverts · Update existing guardrail · Add lighting at paved county road at-grade intersections · Hwy 4 east Jct bridge over TH 60 - Mill and overlay deck, strip seal, update guardrail (if needed), patching delaminated members · CR 27bridge over TH 60 - Mill and patch and overlay deck, strip seal, update guardrail (if needed), patching delaminated members · Construct one cross over and utilize existing cross over to allow 2 lane 2 way traffic during construction; · Detour for duration of box culvert replacement · Right of way for culvert extensions · Agreement with Watonwan County for lighting maintenance 2. Manage the project through letting. 3. Manage the project through construction to closeout. 4. Serve as Project Supervisor during construction.
