Introduction | |||||||||||||||||||||||||
The Change Order is recommended by the Architect/Engineer (A/E) and the Construction Manager (if any), accepted by the Contractor and approved by the Owner. The Change Order formally modifies the Contract. It may authorize a change to the Scope of Work, the Contract price, the time for Contract Completion, or any combination of them. The A/E prepares all Proposal Requests, Change Directives, and Change Orders. All Change Directives and all Change Orders must include the cost and time adjustment of the change. Change Orders must have supportive documentation supplied by the Contractor. | |||||||||||||||||||||||||

The procedure for processing Change Orders may vary depending on if the Change Order is initiated as a Proposal Request or a Change Directive. All methods require completion of the Change Order Estimate Summary. Only the Change Order Estimate Summary form approved by the Facilities Operations and Development will be accepted for processing. | |||||||||||||||||||||||||

1. | Complete all areas shaded in gray. | ||||||||||||||||||||||||
2. | Project Name, Location, Owner (type name of project as it appears on your contract) | ||||||||||||||||||||||||
3. | Contractor [Subcontractor] Name (type name of your company) | ||||||||||||||||||||||||
4. | Contract Number (type the entire contract number as it appears on the contract) | ||||||||||||||||||||||||
For example, Contract No. OSU-120000 / 001 | |||||||||||||||||||||||||
OSU-120000 represents the number for the entire PROJECT | |||||||||||||||||||||||||
001 represents the specific contractor contract type (e.g. YOURS) | |||||||||||||||||||||||||
5. | Change Order Number | ||||||||||||||||||||||||
a. | Contact the A/E [G.C.] and confirm the number assigned to this Change Order. Enter the Change Order Number. |

6. | Complete the Contractor Personnel Costs [G.C. Only; Not Applicable for Subcontractors] | ||||||||||||||||||||||||
a. | List the total number of hours worked, tab over to the rate and type in the base hourly rate and press tab and the total will automatically be calculated. | ||||||||||||||||||||||||
7. | Complete the Labor Detail Summary for each separate labor classification | ||||||||||||||||||||||||
8. | Complete the Fringes for each separate classification | ||||||||||||||||||||||||
9. | Complete the Allowable Payroll Expenses | ||||||||||||||||||||||||
10. | Equipment Rental – Type in the total amount of quotes/invoices to be attached | ||||||||||||||||||||||||
11. | Owned Equipment – Type in the total amount of costs matching documentation to be attached. | ||||||||||||||||||||||||
12. | Trucking – Type in the total amount of your cost for trucking | ||||||||||||||||||||||||
