South West Regional Wound Care Toolkit B.8. NESTLE NUTRITION INSTITUTE MINI NUTRITIONAL ASSESSMENT (MNA) B.8.1. Purpose and Instructions for the Nestle MNA (Mini Nutritional Assessment) Purpose The MNA is a screening and assessment tool that identifies individuals age 65 and above who are malnourished or at risk of malnutrition. By identifying individuals who are malnourished or at risk of malnutrition allows for earlier intervention to provide adequate nutritional support. Instructions The screening tool consists of 6 questions. • Complete the screen by filling in the boxes with the appropriate numbers. • Total the numbers for the screening score. • The screening score (max 14 points): o A score of 12‐ 14 points = normal nutritional status o 8‐11 points = at risk of malnutrition o 0 ‐7 points = malnourished B.8.2 A guide to completing the Mini Nutritional Assessment – Short Form (MNA®‐SF) See PDF document B.8.2.
